1. Schedule Appointment

To see online availability and schedule your visit, click the button below. Make a note if you would like to be put on a waitlist in case of a cancellation for an earlier time or select a different location to see other availability. 2. complete New Client intake forms

Once you schedule your appointment, you’ll receive a confirmation email consent and intake forms electronically and more information on how to prepare for your visit.

3. Verify Insurance

If you have concerns or questions regarding using your insurance and eligibility, please contact us directly by text or the main office number that is listed in your confirmation email. It takes 2-3 days for us to manually verify information, but you can call or text for faster confirmation.
