1. Schedule Appointment

To see online availability and schedule your visit, click the button below. Make a note if you would like to be put on a waitlist in case of a cancellation for an earlier time or select a different location to see other availability. You can also text (727-346-8850) if you are looking for evening or same-day visit specifically.

2. complete New Client intake forms

Once you schedule your appointment, you’ll receive a confirmation email with a link for consent and intake forms and more information on how to prepare for your visit. Please complete consent and intake forms within 24 hours of making your appointment.

3. Verify Insurance

If you have concerns or questions regarding using your insurance and eligibility, please contact us directly by text or the main office number that is listed in your confirmation email. It takes 2-3 days for Deeply Latched to manually verify information, but you can call or text LCA for faster confirmation. If you make an appointment for same day or within 24 hours notice, or haven’t completed your intake form, we may not be able to verify insurance network status and you may owe for the visit- $175 or with financial assistance, $125.
